Survived by her Grandparents Robert and Janet Chestnut of Prescott , AZ Dale A Moscickis and Children: Max, Mickey and Maker Moscickis of Surprise AZ Siblings Ashley Bell of Florida and Wesley Moscickis of Phoenix AZ as well as many other family and friends.

Holly will join her parents Dale and Cinderella Moscickis. Holly will be remembered as a Loving Mother. Per the family request there will be no service. Any contribution can be made to AZ Heart Association https://www.heart.org/en/affiliates/arizona/phoenix
